Über Prof. Dr. med. Christoph A. Maurer

Bei Professor Dr. med. Christoph A. Maurer treffen Sie auf einen höchst erfahrenen Chirurgen, der auf dem Gebiet der Viszeralchirurgie (Bauchchirurgie) als Experte gilt.
Seine langjährige Weiterbildung absolvierte er an international anerkannten, chirurgischen Schulen (Schweiz, Deutschland und USA), wo er selbst viele Jahre eine Führungsposition inne hatte. Seine Erfahrung als verantwortlicher Operateur erstreckt sich auf über 1600 Leber- und Gallenwegsoperationen, 400 Pankreasoperationen, 4200 Dickdarm/Mastdarmeingriffe, 1300 Operationen an Speiseröhre (Oesophagus) und Magen, sowie auf über 700 endokrine Eingriffe.

Prof. Maurer initiierte und leitet das Magendarmzentrum Aare. Es handelt sich um ein Kompetenznetzwerk ausgezeichneter Experten aller Fachdisziplinen rund um das Thema Magen-Darm. Durch die enge Zusammenarbeit dieser Experten profitiert der Patient in besonderem Maße, da sie alle über höchste Expertise im Bereich der Baucherkrankungen verfügen. Prof. Maurer praktiziert dabei an der Privatklinik Obach in Solothurn sowie als Belegarzt an der Klinik Beau-Site in Bern als Teil der renommierten Privatklinikgruppe Hirslanden. Die Klinik Beau-Site beherbergt mit der Abteilung Viszeral Chirurgie eines der beiden wichtigsten nicht-universitären Zentren dieser Art im Mittelland.

Aus dem Curriculum von Professor Maurer geht hervor, dass der komplexen Chirurgie und der Tumorchirurgie an diesen Organen besonderes Interesse geschenkt wurde. Entsprechend verfügt er über diverse Schwerpunkttitel, unter anderem den europäischen Schwerpunkttitel in komplexer Leber-, Pankreas- und Gallenwegschirurgie sowie in chirurgischer Onkologie (EBSQ HPB und EBSQ Surg Oncol). So wiesen der Leberspezialist Prof. Maurer und seine Kollegen im Rahmen einer prospektiven Studie nach, dass große Leberresektionen auch ohne das temporäre Abklemmen der Blutzufuhr (Pringle-Manöver) möglich und sicher durchzuführen sind und sogar Leberschäden reduzieren können. Überdies halten die Operationssterberaten und die Langzeitergebnisse bei Prof. Maurers Tumoroperationen im Bauchraum einem Vergleich mit den publizierten Daten anderer Topkliniken weltweit mühelos stand. Viele Operationen werden minimal invasiv durchgeführt (d.h.endokopisch und somit nur mit kleinen Hautschnitten).

Oft holen sich Patienten und Patientinnen aus dem In- und Ausland bei Prof. Maurer eine Zweitmeinung, z.B. mit der Frage, ob ihr Tumor wirklich nicht entfernbar sei, ob der anale Schliessmuskel wirklich geopfert werden muss oder ob keine Möglichkeit der Verbesserung der Lebensqualität besteht. Speziell in der Coloproktologie (Dickdarm/Mastdarmchirurgie) besteht grosse Expertise, auch in Revisonseingriffen bei unbefriedigendem Ergebnis andernorts durchgeführter Operationen.

Abstract: 
BACKGROUND:
It is not clear whether all patients with rectal cancer need chemoradiotherapy. A restrictive use of neoadjuvant chemoradiotherapy (nCRT) based on MRI findings for rectal cancer was investigated in this study.

METHODS:
This prospective multicentre observational study included patients with stage cT2-4 rectal cancer, with any cN and cM0 status. Carcinomas in the middle and lower third that were 1 mm or less from the mesorectal fascia, all cT4 tumours, and all cT3 tumours of the lower third were classified as high risk, and these patients received nCRT followed by total mesorectal excision (TME). All other carcinomas with a minimum distance of more than 1 mm from the mesorectal fascia and those in the upper third were classified as low risk; these patients underwent TME alone (no nCRT). Patients were followed for at least 3 years. Outcomes were the rates of local recurrence, distant metastasis and survival.

RESULTS:
Among 545 patients included, 428 were treated according to the study protocol: 254 (59·3 per cent) had TME alone and 174 (40·7 per cent) received nCRT and TME. Median follow-up was 60 months. The 3- and 5-year local recurrence rates were 1·3 and 2·7 per cent respectively, with no differences between the two treatment protocols. Patients with disease requiring nCRT had higher 3- and 5-year rates of distant metastasis (17·3 and 24·9 per cent respectively versus 8·9 and 14·4 per cent in patients who had TME alone; P = 0·005) and worse disease-free survival compared with that in patients who did not need nCRT (3- and 5-year rates 76·7 and 66·7 per cent, versus 84·9 and 76·0 per cent in the TME-alone group; P = 0·016).

CONCLUSION:
Restriction of nCRT to high-risk patients achieved good results.

Abstract:

PURPOSE:
This study aimed to investigate in a multicenter cohort study the radicality of colorectal cancer resections, to assess the oncosurgical quality of colorectal specimens, and to compare the performance between centers.

METHODS:
One German and nine Swiss hospitals agreed to prospectively register all patients with primary colorectal cancer resected between September 2001 and June 2005. The median number of eligible patients with one primary tumor included per center was 95 (range 12-204).

RESULTS:
The following variations of median values or percentages between centers were found: length of bowel specimen 20-39 cm (25.8 cm), maximum height of mesocolon 6.5-12.5 cm (9.0 cm), number of examined lymph nodes 9-24 (16), distance to nearer bowel resection margin in colon cancer 4.8-12 cm (7 cm), and in rectal cancer 2-3 cm (2.5 cm), central ligation of major artery 40-97 % (71 %), blood loss 200-500 ml (300 ml), need for perioperative blood transfusion 5-40 % (19 %), tumor opened during mobilization 0-11 % (5 %), T4-tumors not en-bloc resected 0-33 % (4 %), inadvertent perforation of mesocolon/mesorectum 0-8 % (4 %), no-touch isolation technique 36-86 % (67 %), abdominoperineal resection for rectal cancer 0-30 % (17 %), rectal cancer specimen with circumferential margin ≤1 mm 0-19 % (10 %), in-hospital mortality 0-6 % (2 %), anastomotic leak or intra-abdominal abscess 0-17 % (7 %), re-operation 0-17 % (8 %).

CONCLUSION:
In colorectal cancer, surgery considerable variations between different centers were found with regard to radicality and oncosurgical quality, suggesting a potential for targeted improvement of surgical technique.
